A lot of times when you see an unusual pairing of one song "going into" another what you really end up with is one song ending and then the next song starting almost immediately. (Especially in the later years) Every now and then however the Dead create a unique and sometimes beautiful or powerful few seconds of music to take them from one to another. Here are some that I thought of to get started:
74-08-06: Playin>Scarlet>Playin

74-06-23: Spanish Jam>U.S. Blues


76-07-18: Entire second set, especially LiG>Wharf Rat>TOO>St. Stephen


77-06-04:Lots of unusual second set combos but Playin>Franklin's is particularly great

77-12-29: Playin>China Cat

80-06-20: LiG>Althea


83-12-27: Althea>Cassidy

86-03-27: Supplication>Promised Land

Slipping the TOO & Wharf Rat into HSF was an interesting deviation from the norm.

Help On The Way -> Slipknot! -> Drums -> The Other One -> Wharf Rat -> Slipknot! -> Franklin's Tower -> Around And Around

Well '76 is chock full of those as they seemed to have been experimenting with combos, particularly in the summer. Franklin's also got used in a bunch after they dropped H>S>F in the end of '77. Shakedown>Franklin's (11-29-80), Dancin'>Franklins (10-27-79) and as already mentioned the Playin'>Franklin's.

However my favorite is the Eyes>Dancin' transitions that were only performed 4 times. Of the best of these imo is this one:

I also like the one from the Tower Theater run in June '76 but none come close to what Phil does at The Swing.
